Expert Verified: Our family law mediators have checked this is the latest and correct version of Form D81, version 04.25, to use for your consent order application. Last checked against GOV.UK in July 2026.

What is Form D81?

Form D81 is the Statement of Information for a Consent Order in relation to a Financial Remedy. It is the form that must accompany a consent order application in divorce or dissolution financial proceedings in England and Wales. It gives the court a summary of both parties’ financial circumstances and confirms that the proposed settlement is fair and meets both parties’ needs.

The current version is 04.25. Form D81 was substantially rewritten in 2022 to ask for far more detail on income and pensions, and to show the effect of the proposed settlement. The court uses this form to decide whether to approve your consent order without the need for a hearing, so accurate completion matters.

When to Use Form D81

You will need to complete Form D81 when submitting a consent order to court. This applies when you and your ex-spouse have agreed how to divide:

  • Property, including the family home and any other properties
  • Savings, investments, and other capital assets
  • Pensions (whether sharing, offsetting, or no pension order)
  • Ongoing spousal maintenance (or confirming no maintenance will be paid)
  • Debts and liabilities built up during the marriage
  • Any other financial matters that need court approval

Form D81 is submitted alongside your draft consent order, Form A application, and the court fee. Most couples who reach agreement through mediation will complete Form D81 to formalise their financial settlement.

“Form D81 shows the current snapshot of your finances and needs to be as up to date as possible. It is likely the information you disclosed during the mediation or legal process will have changed, even if the actual agreement has not. Our legal packages make sure your information is up to date and accurate before submission.”

How to Complete Form D81

Form D81 asks for detailed information about both parties and the proposed settlement. You will need to:

  • Provide current financial details for both parties (income, assets, liabilities)
  • Explain how you reached the proposed settlement and confirm it is fair
  • Detail any pension arrangements, including Cash Equivalent Transfer Values
  • Confirm whether either party has remarried or is living with a new partner
  • Disclose any significant changes in circumstances since separation
Important: The court will only approve a consent order if it is satisfied the settlement is fair, based on the information in Form D81. Incomplete or inaccurate forms will be rejected, delaying your financial settlement. Our step-by-step guide walks you through each section and includes a worked example of a completed D81 form.
